Forms with various parts independently.

I have created a form consisting of a number of different parts. Depending on what you choose in the previous part - some other parts should be displayed or not. Can this be done? The full form is available to watch here (in Swedish): http://www.jonasn.se/aMazeing/forms/form.php

First part (Delivery): If you choose the first option, the next part is shown, but not if you choose the second option.

Second part (Uniqueness): If you choose the first option, the third part shows but not the fourth. If you choose the second option, the fourth part is displayed but not the third.

Part Five (delivery address): This part will only appear if you select the first option in the first part.

Should we make a database of this? If so, how should the tables be built?
