Forms not functioning properly

I am having problems with two forms, in separate websites. As far as I can tell I have followed the instructions for setting them up, however I have the following issues (code attached): Both forms rely on members logging in via Security Assist.

Form in file labelled 'airshow':
I want this to connect with my database table and prepopulate with id (i will make this hidden once I have it working) Membership #, first name, last name, email, telephone and mobile. I then want members to fill in their aircraft details and insert the form contents into the database as well as auto-email to admin. At the moment, the form prepopulates with the id number and membership number on the first row of the table, regardless of who has logged in. If I remove the bindings to the table columns, it will at least send the auto email, but not insert into the database table (of course).

I want this to connect with my database table and prepopulate with id (which will also be hidden once i have it working) - so far, so good as it is connecting with the correct login details. However, I cannot get it to insert the records into either the database or an auto-email. Also, since I followed the instructions found on a forum about how to repeat the datepicker, there doesn't seem to be any way to bind the datepicker field to the database? Maybe this happens once the form works, but since I can't get it working, I can't tell.

Help please - save my sanity.
