Image Resizer used to work on all images on a page now only works on some

I really appreciate the info and helping me figure this out. It was working before because site had image at 350px but their original was 400px. I changed layout to accomodate a 400px image and was surprised that it stopped caching the images.

Your explanation in this context is clearer to me than the documentation.
