CSS Form Builder and MySQL

I've been tasked with building a form that populates a MySQL database. The data is then to be viewed from a web page which should be editable so items can be deleted once the request is fulfilled. The issue that I'm having is that I just recently upgraded to SuperSuite and, given it's complexity, I'm not quite sure which tools to use. I figured I'd start by building the form in CSS Form Builder. Is there a tool that will enable me to create a MySQL database and insert the appropriate rows, columns, etc. Also, is there a tool that I can use to build a page to view and delete items in the MySQL database? If all of this is possible, what would the workflow be? .
