Error when trying to use CSS Form Builder

When I try to use the CSS Form Builder I get an error concerning the wafb_serial.htm and the shared_ser_mactivation.js files. I have activated using the menu: Insert/Webassist/CSS Form Builder/Client Validation. So I have activated CSS Form Builder but I can't use it when I try to create a new CSS Form ! The path in the attached jpg file doesn't excist. The htm and js files are on C:\Document and settings\....\library and not on drive D: (which is the installation path of Dreamweaver CS4 on Windows XP SP3 system). I've installed and can use all the extensions of WA Super Suite without any problems (except CSS Form Builder). Reinstallation doesn't work either. Who can help me out ?
