Email failing to send due to MySQL errors

I have created a second double opt in system for a new website, and I am having issues with it gooing through the registration properly. The form gets submitted to the DB, but the email triggered from the registration page creates errors. I created the email with dynamic info for the from, cc, bcc and subject being served up from the DB, but I am getting these errors:

Warning: mysql_select_db() expects parameter 2 to be resource, null given in wbesite/webassist/email/templates/Blank.php on line 36

Warning: mysql_query() expects parameter 2 to be resource, null given in website/webassist/email/templates/Blank.php on line 38

Lines 36 and 38 refer to the from email details which are served up by the DB, and the info there is correct. I assume it is possible to have email details such as the from, cc, bcc, subject etc to be served up from a DB because the Create Email wizard allows for dynamic content. The recordsets I've used are very specific and return results as expected when tested in the recordset wizard.

I've attached a zip of the registration page, the Blank.php for you to see, and the live (test) pages can be seen at:

One day, I will get this double opt in thing to work without any issues... I am determined to!
