DFP 2 problems

I use DA 2.1.0 and DFP 2.0.4. I have a simple page that, along with data, uploads a picture to the website. Standard stuff. However, using DFP 2 I cannot get it to work no matter what. If I use older code from DFP 1, it works just fine.

Now I have the issue again when working on an update page with a file field to be able to change the picture. The update page comes up, I select a new picure file, it seems as if it's uploading the picture then the page just goes blank and there has been no upload or changes to any other field. I have followed the PDF about how to do this to the letter I think. I've attached the page as a text file.
