Client wants Form contents to go to a MySQL Data Base

Hi All,

I am a noob when it comes to the WebAssist Super Suite. I created a form for a client on their Volunteer page, which sends the results to the client's email. Problem is now the client wants to form results to go to and populate a MySQL database so they can pull the data at any point. I can not for the life of me figure out how to tell this form to send the contents to a database.

The only post that I could find relevant is this one: showthread.php?t=13848
which doesn't tell me anything about how to do it and the documentation is sparse.

here is the page with the form. volunteer.php

I already created a table in MySQL but dont know what extension to use to have the form contents populate the database.

Any help is appreciated.
