WA_AuthTK_Auth_User - JavaScript Error

Do I have created a simple login system using security assist and my own database with the following user table. The fields are:

UserId, UserEmail, UserPassword, UserNames, UserLevel.

The simple login is only for the administrator of the backend.

When running security assist I deleted the standard suggested pages, registration, user update etc and only left with the login and logout pages. I have saved it as a preset and run it a few times. In my last attempt I've noticed under the manage Pages tab that my login and logout pages locations do not match what is specified at the bottom in the page location field. Screenshot attached.

I am wanting to generate these pages to a directory /ad_min/

After the two pages have been generated I am testing the standard login which is working by redirecting me to the login page with the message stating that I am logged in.

In the server behaviors panel I am double-clicking the security assist authenticated user to adjust the redirect page and to set the user level session.

I get some JavaScript errors with the user interface not been displayed at the end. I have searched the forums for these errors with no results.

This morning I even installed the latest version of data bridge 1.1.2 cleared my cache file and still getting the errors. I am attaching the generated pages and the screenshots of the errors.
