We worked through this in a connect meeting.
1) we added:

if (!isset($_GET['Content']) $_GET['Content'] = 1;
?>
That way there would be a default value for the url parameter when one isn't set.
2) You can get file upload to work properly on update pages by setting the "default value" in the user interface to be equal to the current value from the recordset. Also you have to make sure you are using the "Server File Name" binding from the file upload object when setting the update database column value.
