Registration/login pages not displaying input fields - databridge 1.09

Hi,

I have just purchased the super suite and I have just used security assist to create a login/registation system. It displays correctly when looking at it in live view in Dreamweaver but the input fields will not display when looking at it through a browser any browser infact.

I created the pages using my localhost and have now uploaded it to my webhost. But even so, it refuses to display using either my localhost or my webhost. My web host definately meets the php and sql requirements php is version 5.3.16 mysql version 5.5.27-cll. Here is a link to the login page on my site which is still under construction - login.php

As I said before it will only display in Dreamweaver itself on live view, not any webbrowser. I am new to this so this may perhaps be a simple issue that I don't know about.
