Spry Validation + failed captcha erases form data

I have a form that I used the old form toolkit for validations. Errors popped up in a window. Had to add fields to form. Form worked great before but can't use form toolkit with databridge. Deleted all this validation and added spry validation. If the captcha code fails it takes you to a failed page, when you hit the back button the form is blank and has to be re-entered. I had recreated a shorter simpler form with just a few fields some with spry validation. Failed captcha does same thing, erases anything with spry validation other fields however retain their entries. Searched the forum and didn't find what I was looking for. Can you point me in the right direction to correct this? PHP form file attached in zip format. Other parts of form are datepicker, universal email, add record to database, all of which I couldn't have done without my Webassist tools, so thanks for that! The form is live on registration.php. New error cropped up, don't know from where or why, in my connection script. Guess I need to fix that as well.
