Problems With Captcha Image

Hi everyone,

I am working on a new site which is a bit like a blog where visitors can comment on posts etc.

In order for all visitors to be able to comment, I have added a form via a WA plug-in called form_comments_insert.php (form_comments_insert.php) which has the insert, email notification and captcha function on it.

The form looks great on the page when plugged into the post page: episode.php?id_post=2

However, the captcha image does not appear. I've made sure all the files are uploaded to the server and that the GD library is enabled. But still no pictures...

Also, when testing the form and adding random text into the captcha box (because I can't see the picture), I get this error message: Warning: Cannot modify header information - headers already sent by (output started at /home/hantsle/public_html/podcasts/episode.php:4) in /home/hantsle/public_html/webassist/form_validations/wavt_scripts_php.php on line 84

I can't figure out what's wrong. Any ideas?
