Form Builder issue

I have been starting on a blank page - completely blank. However, I have now just discovered that the problem only occurs when I click the formbuilder icon in the webassist icon panel. If i go webassist > forms > insert form, the issue doesn't arise and the formbuilder wizard starts normally. This may explain why you couldn't replicate the issue. I will go ahead and now make a form to see if any further errors occur
