How to get started with relational database

I have the need to understand "how to get started" with "terms" that a newbie can understand please?

I would like to create a Relational Search whereby the end result will be either a:

  1. web-page form or...
  2. a .pdf form

The Primary level search would be for "Bond" and the search criteria would be first in this order:

  1. In what State do you require the Bond?
  2. What Class of Bond do you require? (This is drop down menue or dynamic text...examples would bid bond, payment bond, license bond)
  3. What Category of Bond?
  4. Obligee (relational to State)

Then, upon submit... there would be either a "web-page" with "web-form" or a specific .pdf form displayed.

As I think about this it could be set up kind of like a shopping cart?

There could also be some conditionals in here such as have you ever had a bond declined?

Or, Have you ever had a claim?

Then depending on their response " Yes / No" the "web form" or "pdf form" would display.

And, if anyone knows "lifecycle designer?" and would know how to dynamically link a web form data to a .pdf form...cudos!

I hope this makes sense.

I would even be willing to come up with some money to have someone show me "slowly" how to do such a task. Money is tight so I am Johnny poor boying it right now
