MySQL Error#: 2000

I'm getting a MySQL Error#:2000 when I try to define the PowerCMS Builder MySQL connection. see attached screen shot. I did not set up the database but the info was given to me.

Is this something I'm doing or is it on the database side of things.

If this helps, here is the software version:
Software version: 5.0.41-community-log - MySQL Community Edition (GPL)
