What other extensions are installed, including version numbers?

Does this error happen when creating a recordset? what are the steps you are taking to reproduce the error?

try creating a new blank php file, does the error happen when using the blank PHP File?

If not, it is most likely being caused by a code chunk on the page, send a copy of the page so i can inspect the code.
