sounds a code order issue.

Before Proceeding, make a backup of the confirm.php page.
In the server behaviors panel, select the Universal Email server behavior. This should appear after the eCart Store Order Summery server behavior. If it is in the list before the store order summary server behavior:

Select the universal Email server behavior. I code view, the Universal Email code will be selected, Cut this code. If a blank space is left between to code blocks, delete it.

Now select the Store Order summary server behavior, this will highlight all of the store order summary code, find the end of the highlighted code and paste in the Universal Email code.

Most email clients wont show the image by default until you click a Display Images button. If this still doesnt show the images, edit the email body. make sure the source fir the images is fully qualified URL to your server, for example:

php:
<img  src="http://www.mydomain.com/images/<?php echo $cartName->DisplayInfo("ImageColumn"); ?>" />
