errors, errors, errors.

I have now arrived at the end of my tether with your extension form builder after purchasing it with data assist last Monday at a princely sum of £300… I have got nothing done since, losing lots of time and money! Firstly data assist generates blank pages (triangle_one_Insert.php ) some pages work; update and details and some don't; search etc and for form builder it won’t even start up giving errors starting with the memento error as i start Dreamweaver cs5 then when adding a form the argument 1 error followed closely by the line 5085 error resulting in a damp squib (images attached) please help as I can’t afford any more wasted time on this. Please don’t ask me to delete the cash or reinstall because I have done this numerous times.
