Issues with CSS Form Builder

I have version 2.00 installed. In fact, i recently re installed and reactivated to start over. I've created a .php page and attempted to insert a simple contact form.

initially, the form inserted and has large bold red font with the form titles and when i uploaded this page, i received this error message:

Parse error: syntax error, unexpected ')' in /home/golfina1/public_html/contact.php on line 157

The page is contact.php

Also, when i attempt to re insert the form into a new page, before form builder starts i get the following message:

"while executing onload in wafb_addform.htm, teh following javascript error occurred:

in file wafb_addform initialize ui is not defined.

As a follow up, i deleted the cache again, and it now lets me create pages and i'm now getting this error message:

"Fatal error: Call to undefined function virtual() in /home/golfina1/public_html/contact.php on line 1"


Would prefer an opportunity to discuss via telephone support.
