Changing Configure Field Settings - Javascript Errors

Security assist 2 errors

I have followed the tutorial for using security assist 2. The first step works fine using my own database user table and user level table. I then click on manage pages tab and add a prefix to the pages name e.g. a_login.php as well to the other remaining pages.

When I click on Configure Field Settings and select a database field I get a host of errors, namely:

Error in populate.initFormBuilder:
TypeError:fieldObj has no properties

Error in populate.properValidationSub:
TypeError:this.src.lines[opts.lineIndex - 1] has no properties

While executing onChange in SecurityAssist_SubFields.htm, the following JavaScript error(s) occurred:

At line 1235 of file "C:\users\Frank\AppData\Roaming\Adobe\Dreamweaver CS 5.5\en_US\Configuration\Shared\WebAssist\SecurityAssist\fb_shared_functions.js":
this.src.lines[opts.lineIndex - 1] has no properties

I have cleared the cache, and install the extension, reinstall the extension. Same errors. I then uninstalled the following extensions: form builder, HTML editor 3, and again security assist 2. Then deleted: WinFileCache-9287E01C.dat. Rebooted PC and reinstalled in sequence, security assist 204, CSSFormBuilder201_SR2 and lastly HTML editor 302.

Still getting the above errors.

I found a thread that discuss the sequence of extension installation. I therefore uninstalled: Form Toolkit, Form Builder, HTML Editor and security assist. Deleted cache file. Rebooted machine and installed extensions in the above order.
