Thanks Jason. That did format the date correctly but the error message is an enigma:
Incorrect datetime value: ''2011-11-07 12:00:00'' for column 'monitorGoLiveDate' at row 1
An example of a datetime value that inserted properly before I added the jQuery widgets looks like this:
2011-10-13 13:06:29
