multiple dynamic slect lists on item page

I'm trying to include 3 drop down lists on each item page that pull from the same set of data (alphabet). I want each of these 3 options to show up on my cart/order.

I've tried several ways of doing this and could never make it work past adding the first select list.

Really, what I would like to do is have a separate table with all my options to be available to every item. I suspect I need to create 3 columns, create an updatable select list for each of them using the add to cart dialog, then call for them in the cart. Did that - the item won't add to the cart.
