Radio buttons

I am trying to build a simple form with 4 radio button groups with just 2 buttons in each group with Yes and No as the labels and values.

I have gone through the Form Builder interface and setup the Form Elements but once the group is in place I can't seem to get more than 1 button label to show and there are no actual buttons visible when I preview in the interface or in a browser.

I don't find anything in the support guides for Form Builder that goes over using radio buttons or check boxes. Can you ive me a hand with this?
