Cannot open CSS Menu Writer Manager

I am on Vista, using Dreamweaver CS3, running CSS Menu Writer v2.01. When I try to open the Menu Manager I get the following error message:
"While executing onLoad in walm.htm a JavaScript error occurred." I then click the OK button on the error dialog box. This message appears again; I click OK again. I then close the blank Menu Manager window.

I have uninstalled then reinstalled the extension, CSSMenuWriter201.mxp. No difference. I've shut down and restarted my PC. No difference.
