Mail not sent when using a form as a dynamic plugin

I have followed the video tutorial for Framework Builder. The google map (static plugin) and the forms (dynamic plugin) are working as expected.

I have also applyed email functionality to the forms. The default form (Send us a message) is workin as it should. Mail is sent and the user is redirected to the "thank you" page as he should be.

For the second form (Ask us a question) the mail is not sent and the user is not redirected to the "thank you" page. (No error messages. The user is redirected back to the contact page)

Email for both forms are set up the same way and the "Send us a message" page and the "Ask us a question" page are working when I test the plugin pages one by one.

The problem is that the "Ask us a question" page is not working when included as a dynamic plugin.

You can test it on live URL here: contact.php

I'm also attaching my working files.
