Search Wizard

I have created a results page and a search page, and they both work fine. However, when I use the Data Assist Search wizard to add search options to a separate page, I'm running into problems. Everything works well in dreamweaver, but after I upload to my server the results page no longer loads at all for either search page. My browser says it is 'Done', but it has loaded a completely blank page.

Is there a problem when linking to separate search pages to a single results page? I'm thinking there's something I'm missing, but I've messed around with it quite a bit and can't seem to come up with anything. Any help would be appreciate. Thanks.
