Good morning,
After a late night trying to resolve this issue, I am afraid I need to ask for help.
On a different post not related to ccs form builder someone else was having a similar issue.

Warning: chdir() [function.chdir]: No such file or directory (errno 2) in /home/lanwide/public_html/webassist/email/mailformatting_php.php on line 140

Warning: require(Blank.php) [function.require]: failed to open stream: No such file or directory in /home/lanwide/public_html/webassist/email/mailformatting_php.php on line 142

Fatal error: require() [function.require]: Failed opening required 'Blank.php' (include_path='.:/usr/lib/php:/usr/local/lib/php') in /home/lanwide/public_html/webassist/email/mailformatting_php.php on line 142

Both the file and directory exist on my my server. I confirmed this with file zilla. Also deleted the files and re did the forms and uploaded several more times.

This is what I get if I use a blank page with the current working directory command
The Current Working Directory is: /home/lanwide/public_html

I change the permissions but still can't make it work.
Any help would be much appreciated.
