Pages show up blank when published

Jason,

Attached are zip files for the page and the universal email webassist folder. I do not have a seperate Universal Email folder; however, I do have several pages on the site that incorporate Universal Email. Let me know what else you need, as I am anxious to get this fixed. It's been about a month that I've been battling this. Thanks for your help!
