You can make the html form for it first, then once you create the form you can just add it into the html code on the page wherever you would like to see it. If you want it to work the same as another login you have you will need to make it post to the same login page that has the login server behaviors on it.

You need to get the form created first though. If you need any help post back with your login page and the page that you would like to add a small version to and I could try to create an example for you.
