Tax rule not working...

I have an active tax rule in the cart object to charge MD sales tax if the shipping address is in MD. This rule is not firing off at all.

I had additional rules for government exempt cards. We had a customer from MD purchase an item, but no tax was charged. We verified that the card was not govt issued, and I began stripping out the rules to get to simple MD tax.

Old rule:

It is still not working.

The click chain in my cart is checkout (enter payment info) > confirm (submit to Authorize.Net) > success.
