the first thing I would do is add redirects for the look up success and lookup fail in the email password server behavior. make sure to use different pages for the success and fail redirect so you can gauge whether the email is truly being sent.
If it is redirecting to the success page, and you still do not get the email, there are 2 possibilities.
1) The email is being caught in a spam filter, check your spam folder.
2) The smtp server is refusing to relay the email for some reason. It may be that the smtp server will not relay emails for that from address you are using. It may be that the SMTP server will not relay email to the to address you are using. To find out why the email is not being relayed, you will need to contact the host. They will need to examine the smtp servers error logs to see why the email is not being relayed.
