Simple form with Paypal Integration

Hi, I have my client who offers diet advice to people. He wants to have a simple from filled out with the user questions and sent to his email along with a check out payment on his paypal account.What is the best way to achieve this.He wants to track all the transactions including payments and the user email who placed the order. Please enlighten me with ideas
