In Universal Email, you probably need to rebind those forms elements in the email message since the names have been changed.

When you first created the email message, the code to output the form values would have been created based on the old names of those elements. now that the names have changed, you will need to edit the email message and add new bindings for those form elements so the code will be generated based on their currant names.
