Form difficulties

Jason,

Ok I think I'm on the right track following your suggestions BUT when inserting Universal Email into my PHP page. I fill in the Email object, General & From Tab and try to click "OK" but UE asks me to add a recipient to the email. Would I put my email in there or what ? It's confusing me since at the least I want my form to send the info input on the form to my email address and at most CC them the info they put in to remind them.

Regards,
Impeccable
