Form submit problems

cannot get PHP Solution pack working

I followed the instructions on the getting started with contact form pack (I purchased this since for w/e reason Universal Email is grayed out in the Insert-WA menu) I modified the WA_global.php file's attributes to point to my email address, my form points to contactus.php and I still get the error here: contactus.php
I'm attaching the files affected to help whoever assists me....Thanks in advance..
