Need step by step for Contact Form

The structure of my site

Here's a link to my most current form. This one works and sends the email as required.

contactus.php

Just needs to be restyled, like the last one you posted, neilo. Centered form, upper and lower case, only including the captcha security code, not the second one. I'm gonna be trying to accomplish this on my own today (on a duplicate version, not this one that works), and I will post if i get it to work. But if you get this message before you see a new post, definitely let me know how to do the styling.

The structure of my site is the same as your last posting with the image attachment so I think that's all good.

I am kinda embarrassed that the whole problem with the email not sending was due to my own ignorance and inability to follow direction correctly. As mentioned, I was changing a line in the contact_us.php that resembled the line called out to change in the WA_globals.php. The only reason I discovered it was because I was searching for the line to change the email server in the contact_us.php, and it was not there. So I went back and reread the instructions, and I'm like, "DUH!!!!" What an IDIOT I am! Changing the correct file makes a huge difference.
