Are you planning on having flat rate shipping based on the options the user chooses? If so you can just add in a select list for the types of service on the checkout page. Then on the confirm page you will need to set a session variable to the value of the service select list.

Once you have set the session variable you then can create flat rate shipping charges based on the value of the session variable you have set. This is kind of just a quick overview of how you could add in flat rate shipping with selections like this. Please post back and include some more detail about how you would like the shipping charges calculated and the values you will be working with for the shipping.
