Attachments not working, HTML editor required

I created an email form per my boss's request. The tools are great, it sends the email and goes into the database like any other form using the Webassist functions. I used the HTML editor for the body message as well. Great stuff. The one thing I keep getting is an error for the attachments.

Notice: Undefined index: in /home/public_html/contacts/mailmgr/emailform/cacmail.php on line 61

Line 61 is

$Email->addAttachment("attachments_folder/".$WA_DFP_UploadStatus["WA_UploadResult1"][""] ."");

(The attachment does get uploaded though).

And..what is the way to make the HTML Editor 'required', since it's not an <input type 'text' field.
