The issue currently is that the prodtype table has a primary key column defined that is not set to auto-increment. I think if you update that table definition in phpMyAdmin it will fix the problem. (Please download the latest version of the page. I made some other fixes like setting the checkbox values and updating the server behavior so it won't insert when the checkbox is unchecked)
