Captcha element does not validate when the correct code is entered

Hi Ray and Team:
I'm working on an HR application upload page on our site that has a CSS FormBuilder on it. It was working fine until my HR people asked about uploaded applications. When I tested the page, I got the error: "Entered text does not match; please try again" even though the submitted string in the Captcha field was correct. I made sure that my Captcha image and input form have the same id value. In checking the <img> code I found that one of the parameters was "&field=Security_Code_1" so I made sure that my Captcha input form was named the same and set the <img id="Security_Code_1"... /> as well. This allows the form to pass validation, but instead of returning me to the confirmation page, it returned me to the form page. The URL for the page includes "?invalid=true" in the querystring.

Checking my directory where the file should have uploaded, I did not find the file, so apparently the form is not working properly and I need some pointers as to how to troubleshoot this issue.
