show all rows from database table as default

Thanks Ray, that solved the error. I now have all rows of Data showing as default. But I still cant get the form and a-z urls to interact with the data search behaviour. How do I do this. I have tried adding an item for each of the select fields in the form, both with 'List' filter types and binding the appropriate column from the form data. Also for the a-z URLs I have tested A for 'first-name' and B for 'surname' using 'Value' filter types. But it just isn't happening. ( see attachments )
