Upload stopped working; DFP won't activate

I've had DFP file upload working on my website for over three years and now it has stopped working. The progress wheel now just spins and spins.

I then went to DW4 to create a test upload page but ran into problems there. When selecting File manipulation -> Upload, I get the following DW error:

While executing onLoad in WA_DFP_FileUpload.htm, the following JavaScript error(s) occurred:

At line 95 of file "C:\Program Files\Adobe Dreamweaver CS4\Configuration\Commands\WA_DFP_FileUpload.js":doSerialCheck is not defined

So bottom line on this is that the function is no longer working on my site and the extension is not working in DW4.
