In you screen shot, it looks like you have a Database column named "When?". this is not a legal column name. you should not use punctuation in your column names. even without the punctuation, the name "when" is an internal MySQL function is considered a MySQL reserved word. MySQL reserved words should be avoided as column names. see the following for a full list of reserved words:

In your Dreamweaver Site definition, check the Local Info tab and make sure the "Create Links Relative to" Option is set to "Document" instead of "Site"
