A function that worked in another form doesn't work in this one & I get multiples of the info emailed

I need another person to look at a form that I have created because I can't seem to solve the problems I'm having.

1st: A function that I used before to " return validateInputs()" when submitted is not working.

2nd: when the form does submit it sends multiples of the information entered so instead of a 2 pages email it's 78 pages of the same email response over & over again.

Could you help me where the errors are so I can fix them?
