Trying to update a CSV file with form data

After using your tutorial in Data Bridge for universal email (the appointment request form), I am managing to receive the emails fine with the form data. The other thing i need it to do is update a CSV file with headers and field data. I have added the attached code. For some reason, multiple blank results are getting written into the CSV file. Can someone explain why this may be?..

to explain the attachments,I have a webassistform.php which calls on the writetocsv.php to create the form-data.csv file.

when i open the CSV file after submitting the form, it reads like this:
