Insert New Form with Data Bridge

Firstly.. I now realise my error with the 'From Address' which has now been corrected.

The SMTP server 'smtp.teampropertymanagement.co.uk' was just me testing other servers we have hosted to see if they work but they didn't.

One thing that is worth mentioning is that I have a fully working form using exactly the same settings as the one I am trying to create. this form is on the Public site and was created a couple of years ago on an earlier version of your plug-in. View the form here: http://www.rtmf.org.uk/contact-us.php and compare it to the test-form at test-site.rtmf.org.uk/contact-test.php. IS THERE AN OBVIOUS DIFFERENCE BETWEEN THE TWO? The working form is using info@rtmf.org.uk the test form is using joiner@rtmf.org.uk which has also been tested successfully on the working form.

Something else worth mentioning is that quite often when click the 'Envelope Icon' to enter the 'Configure Email Settings' window it Crashes Dreamweaver. This happens regularly.

Is this a known issue or am I suffering from a CORRUPT download of the plug-in?

Something else that was strange was when I was copying the files from the testing server and pasting them into the 'test-site sub-domain folder' the email settings were blank, even though I had set these before copying. I then thought this was the problem so re-entered the settings and then uploaded to the remote server, incidentally i then got the message I have attached as screenshot.

Still though the form wasn't working.

I am currently on a brand new iMac with an up-to-date version of Dreamweaver CC. I have lost count how many times I have un-installed and re-installed your plugins to solve issues but unfortunately this has no effect.

I have now run out of options on what I can do to solve this.
