Who Is your Host?
In the email settings, you have used the Form input for the from address, many hosts will require that the from address be a valid email address for your domain
In your form page, open the server behaviors panel, and click the the Universal Email server behavior to edit it. Click the from button to go to the advanced from settings, Use a valid email for the from address for your domain, in the replay to setting, you can use the dynamic binding.
depending on your host, you may also need to check the option to include the from address as an extra parameter.
In the UE server behavior you can also set the go to page setting to redirect to the thank you page.
If you continue to have a problem, send a copy of the form page (compress the php page to a zip archive) and a link where i can test the page.
