who is your host? depending on your host, you may need to enter an SMTP Server address and port. your host may also require that the option to send the from Address as an extra parameter be selected

What did you enter for the From Address? most hosts will require the From address to be a valid email address for your domain.

What about the To Address?

Did you enter a go to page in the universal email behavior?

if so, does it redirect to the go to page?

Are you using Data Bridge to create the contact form?

Do you have the most recent version (1.1.3) installed?

it would help to have a copy of your page and the webassist/email folder to be able to see the code.
