Two issues with new Design Extender Install re file uploads

Hi I have two issues going on. This is an upgrade install from Power CMS 1 to the Design extender CMS.

First, when I try to upload an image and hit the "Browse Server" button, I get the following error:
alert("MDB2 Error: no such table\nSELECT extension, plugin FROM kfm_plugin_extensions WHERE user_id=1")

The next issue is, should there be an "Upload/Insert" button in the editor? My client needs to upload mp3 and wav files and all I see is an "image upload" button.

Would appreciate help as soon as possible since this is a live site.
